Abstract: This qualitative study investigates the experiences of 20 inpatient pharmacists in managing high-risk medications, including anticoagulants, immunosuppressants, and chemotherapy agents. Through semi-structured interviews, pharmacists' strategies, challenges, and collaborative practices in medication management within hospital settings were explored. Thematic analysis identified key themes such as medication optimization, interdisciplinary collaboration, and professional development needs. The findings highlight the pivotal role of pharmacists in enhancing medication safety and patient outcomes. Recommendations include tailored education and support to equip pharmacists in effectively navigating complexities associated with high-risk medications.
