Abstract: This qualitative study investigates the challenges pharmacists face in managing medication adherence among elderly patients and the strategies they use to overcome these obstacles. Fifteen pharmacists were interviewed to explore barriers such as complex medication regimens, cognitive impairments, and socioeconomic constraints. The study also examined strategies employed by pharmacists, including regimen simplification, patient education, use of adherence aids, and interprofessional collaboration. Findings reveal that while complexity and cognitive issues are significant barriers, pharmacists utilize various approaches to improve adherence and support elderly patients. This research highlights the critical role of pharmacists in enhancing medication adherence and offers insights into effective practices for addressing adherence challenges.
